AADL Bed Recycle Program process

Please note: If your client requires a bed through AADL, they should contact their Health Care Provider
either through the Hospital or Home Care Services to arrange for a Home Care Bed.

To submit a Bed Authorization, please fill out the Bed Depot Order Sheet below.
Please Fax or Email this form to Eco Medical Equipment in addition to
submitting Authorization via the Alberta Blue Cross Portal
