AADL Equipment Recycle Program

• NOTE: Please do not return equipment without first contacting the Recycle Department and receiving confirmation.
  

To submit a new request for recycle equipment or return used equipment, please email us with the following information:

For New Recycle Requests: 

• Contact Name & Phone Number
• Email Address
  
• Delivery Address
• Client Name, Date-of-Birth, and Personal Health Number (PHN)
  
• Model Type, Size, and Specifications
• Relevant Generic Specification Form (as listed on Alberta Blue Cross)

For Returning Recycle Equipment No Longer Needed:

• Client Name, Date-of-Birth, and Personal Health Number (PHN)
• Pick up Address:
  
• Contact Name & Phone Number & Your Name
• Equipment Type & Serial Number
  • For Manual Chairs – Is this a Non-Folding or Folding Chair?
  • For Power Chairs – Is it Driveable? Can it be put into Manual Mode?
  • For Patient Lifts – Is it a Ceiling Lift or Floor Lift? Does it fold?
• Are there any additional pieces of equipment (with a Government of Alberta Not for Resale sticker) that you want to return?

• Authorizers submit generic specification forms
• Eco Medical Equipment searches the recycle inventory and sends a “match or no match” response
• For Matched equipment, Eco Medical Equipment will create an Estimate with AADL approval and send it to the Therapists
• The Therapists are responsible for confirming cost-share and upgrade amounts with the clients
• Once the Estimate has been approved and reviewed, Authorizers must submit authorization to ABC and notify Eco Medical Equipment by providing the ABC Reference#
• Eco Medical Equipment is responsible for collecting cost-share, completing refurbishment, and delivering equipment to clients/vendors
• Therapists are responsible for following up with clients to ensure equipment is set up, adjusted, and working properly

• Select “Wheelchair, seating, and recycle equipment providers”
